How to Apply
- Contact a CACFP representative by phone at 631-462-0303 x116 or email firstname.lastname@example.org.
- The representative will complete a pre screening questionnaire with you over the phone and follow-up with an email requesting for additional information.
- Once the representative has received and reviewed the requested information, they will schedule a preapproval visit to your home. This visit includes training for you and any assistant on all program requirements.
In order to become a CACFP provider you are required to have at least one child in care that is not your own, a copy of your license, and you or anyone in your program is not on the National Disqualified List.
