Thank you for your interest in participating in the Child and Adult Care Food Program (CACFP) and providing healthy and delicious meals to the children of Suffolk County! 

How to Apply

  1. Contact a CACFP representative by phone at  631-462-0303 x116 or email cflythe@childcaresuffolk.org.
  2. The representative will complete a pre screening questionnaire with you over the phone and follow-up with an email requesting for additional information.
  3. Once the representative has received and reviewed the requested information, they will schedule a preapproval visit to your home. This visit includes training for you and any assistant on all program requirements.

 

In order to become a CACFP provider you are required to have at least one child in care that is not your own, a copy of your license, and you or anyone in your program is not on the National Disqualified List.
